Job summary

Monarch Communities is seeking a Care Manager (CNA) for a full-time, day shift (6:30a-2:30pm) in Delaware. This non-exempt, hourly role reports to the Clinical Services Director and involves assisting residents with daily living tasks per care plans while upholding safety and dignity.

The position requires a Certified Nursing Assistant or DHS certification with a High School Diploma; dementia experience is a plus. Benefits include health and life insurance and a 401(k) match.

Qualifications

• Certified Nursing Assistant and/or DHS certification plus High School Diploma
• Knowledge and experience in working with the elderly population, preferred dementia experience

Responsibilities

• Provide resident care including bathing, dressing, toileting, and feeding per care plan
• Maintain safety, dignity, and confidentiality of residents
• Escort residents to meals and activities
• Ensure residents are clean, dressed, safe, and fed during shifts
• Document all services provided during shift
• Report any change in condition to supervisor
