Job summary

Monarch Communities is seeking compassionate caregivers (Care Managers) to assist residents in daily living per care plans in an assisted living/memory care setting. The role is part-time (less than 30 hours per week) with shifts including 1st and 3rd shifts, paying $19.00–$22.00 hourly in Southbury, CT.

We require a High School Diploma or GED, 1 year of resident care experience, and CNA or CHHA certification. Benefits include PTO, flexible schedule, on-the-job training, EAP, and free parking.

Responsibilities

• Escort residents to meals and activities when needed.
• Document all services provided in the EHR during shift.

Job Description

Led by the community's Director of Health and Wellness, we are looking for compassionate and dedicated caregivers (Care Managers) to assist residents in all aspects of their daily life as indicated in the resident’s care plan, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities.

Schedules

• Part-time position (1st and 3rd shift openings) in Assisted Living and/or Memory Care
• Salary Range: $19.00 - $22.00 Hourly

Responsibilities and Duties

• Assist with self-administration of prescribed medication to resident and maintain related medical records, by way of queuing, prompting, and/or reminding; under the supervision of the RN and/or LPN.
• Provide resident care to residents that include bathing, dressing, toileting, and feeding, as outlined in their care plan
• Respects the needs of team members and residents by adhering to schedule and documenting attendance properly.
• Maintain the safety, comfort, dignity, and confidentiality of all residents
• Encourages and escorts (when needed) residents to meals and activities
• Ensures all assigned residents are clean, neatly dressed, groomed, safe, and fed during assigned shifts
• Document all services provided in EHR for your assigned residents during your shift
• Monitors and reports any change of condition in a resident immediately to your supervisor
• Appropriately utilizes and keeps electronic documentation “devices” safe and secure within the community; efficiently documents care/services provided during the shift.
• Participates in and attends all in-service trainings as scheduled
• Ensures compliance with all Federal, State, and local regulations, as well as community policies and procedures
• Provides basic housekeeping and/or laundry services when needed
• May be asked to obtain weights and vitals of residents and document same per RN request
• Other duties as assigned by supervisor, RN, and/or ED

Qualifications

• High School Diploma or GED. At least 1 year resident care experience required.
• Certified Nursing Assistant or Certified Home Health Aid
• Knowledge and experience in working with the elderly population, preferred dementia experience

Physical Abilities

• While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
• An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.
