UnitedHealth Group is seeking a Patient Service Coordinator to support home health operations, serving as a liaison among patients, caregivers, clinicians, and office staff. This on-site role in Hawthorne, NY runs Monday through Friday, 8:00 a.m. to 5:00 p.m.

Qualifications

• High school diploma or GED required.
• 1+ year scheduling experience in a healthcare environment.
• 1+ year administrative experience in a healthcare environment.

Responsibilities

• Schedules patient assessment visits including Oasis visits in a timely manner.
• Allocates clinicians to visits across geography to maximize efficiency.
• Collaborates with field clinicians to ensure visits are completed as ordered.
• Manages inbound calls from patients and staff regarding scheduling issues.
• References issues to the clinical manager as needed.
• Monitors hospitalized patients and communicates follow-up actions.
• Supports timely communication with patients, clinicians, referral sources and office staff.
• Runs reports and responds to workflow with appropriate actions.
• Assists with transfers of patients between centers and/or hospice services.
• Maintains supply closet and coordination with field for patient supply needs.
