Job summary

Enhabit Home Health & Hospice is seeking a scheduler to align patient needs with field clinicians and ensure seamless care coordination across teams. You will work with patients, physicians, referral sources, caregivers, and staff to maintain continuity of care.

The role emphasizes on-site collaboration, accurate communication, and day‑to‑day management of scheduling and care logistics to support high‑quality home health services.

Qualifications

• High School Diploma or equivalent.
• Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
• Current state clinical license as required by state rules.
• At least 1 year of clinical experience in a healthcare setting.
• CPR certification is required.
• At least 1 year of home health, hospice, or pediatric experience within the last 24 months.

Responsibilities

• Schedule patients to field clinicians.
• Coordinate care by communicating with field staff, patients, physicians, referral sources, caregivers, and other providers.
• Maintain on‑call notebook and hospitalization logs to improve communication among stakeholders.
