Job summary

UnitedHealth Group is seeking a Patient Services Coordinator - Clinical to schedule home visits for field staff and coordinate patient care under the Clinical Manager. You will help maximize care center capacity by building efficient clinician schedules and addressing scheduling needs with patients and staff.

The role requires an active RN/LPN license, healthcare scheduling experience, and willingness to travel. It offers competitive hourly pay and a comprehensive benefits package.

Qualifications

• Active, unrestricted LPN/LVN or RN license in the state of assignment.
• 1+ year of administrative experience in a healthcare setting.
• 1+ year of scheduling experience in a healthcare setting.
• Willingness and ability to travel as required by the business.
• Must possess a valid driver’s license, reliable transportation and auto insurance.

Responsibilities

• Schedule patient assessment visits, including Oasis visits, in a timely manner.
• Prepare and monitor weekly clinician schedules to optimize capacity and productivity.
• Assign clinicians efficiently by geography and care needs.
• Schedule missed visits or escalate to Clinical Manager for resolution.
• Coordinate with field clinicians, patients, referrals, BD team and office staff.
• Manage calls from patients and staff regarding scheduling issues and refer clinical issues as needed.
• Monitor hospitalized patients and ensure follow-up by the team as required.
• Process orders and services in schedules (lab draws, visits, add-ons, etc.).
• Provide timely communication with patients, clinicians and care team; run relevant reports.
• Assist with transfer of patients between care centers and/or Hospice services; may provide direct care in emergencies if licensed.
• Maintain supply closet and liaise with field for patient supply needs.
• In absence of Intake Coordinator, assist with referral and intake within the care center.
• May obtain verbal orders from licensed clinicians when appropriate.
• Performs other duties as assigned.
