Job summary

UnitedHealth Group is seeking a Patient Service Coordinator to support home health operations as a liaison between patients, caregivers, clinicians, and office staff. This non-clinical role coordinates scheduling, manages administrative processes, and ensures timely communication to support quality patient care.

The successful candidate will schedule Oasis visits, assign clinicians efficiently, handle calls about scheduling, and collaborate with the clinical manager and field staff to maintain

Qualifications

• High school diploma or GED equivalent.
• 1+ years of scheduling experience in a healthcare environment.
• 1+ years of administrative experience in a healthcare environment.

Responsibilities

• Schedule patient assessment visits including Oasis visits.
• Assign and schedule clinicians by geography to maximize efficiency.
• Collaborate with field clinicians to complete visits as ordered.
• Manage calls from patients and staff about scheduling.
• Refer clinical and performance issues to the clinical manager.
• Monitor hospitalized patients and coordinate follow-ups.
• Communicate with patients, clinicians, referral sources, and office staff.
• Generate reports and take actions based on workflow.
• Assist with transfer of patients between care centers or hospice services.
• Maintain supply closets and liaise with field staff for patient supplies.
• Perform other duties as assigned.
