Job summary

GT Independence seeks a bilingual Case Manager in the San Francisco area to coordinate Medicaid Waiver services and promote participant independence. The role is on-site and involves collaboration with staff, families, and regional centers to ensure quality service delivery.

You will manage documentation, monitor safety, provide training, and support community integration while traveling for events as needed. Competitive wages, benefits, and growth opportunities are available.

Qualifications

• Be 18 years of age or older.
• Have current, appropriate licensure.
• Master’s degree in social work, Psychology, Counseling, Rehabilitation, Nursing Gerontology or Sociology and one year experience with the population; OR Bachelor’s degree and two years’ experience; OR RN/Associate degree with three years’ experience.

Responsibilities

• Promote personal growth, independence, and quality of life for service participants through initial development and ongoing support, monitoring, supervision, and staff training.
• Build and maintain positive relationships with agency staff, service providers, and individuals/families.
• Ensure accuracy, quality, and timeliness of all documentation in compliance with state and agency requirements.
• Travel will be required to attend events, conferences, training, and other related activities.
