Job summary

GT Independence is seeking a dedicated Support Broker to coordinate daily Medicaid Waiver services and promote participants’ growth and independence. You will collaborate with staff, families, and community partners to align with Individual Support Plans, while handling documentation and regulatory compliance.

This role offers opportunities to travel for events and training, with a mission-driven environment that values autonomy, trust, and meaningful community connections.

Qualifications

• Minimum requirements include 18+ years old and current licensure.
• Master’s degree or equivalent professional background with experience in the population.
• RN or related degree with experience also acceptable.

Responsibilities

• Promote growth, independence, and quality of life for participants.
• Build relationships with staff, providers, and families.
• Ensure documentation meets state and agency requirements and timelines.
• Travel to events, conferences, training, and related activities.
• Deliver presentations to Independent Facilitators and external stakeholders.
