Job summary

GT Independence is seeking a dedicated Support Broker to coordinate daily Medicaid Waiver services and promote participant growth and independence in line with district regulations and GT policies.

The role requires building relationships with agency staff and families, maintaining documentation, and guiding participants through ISP outcomes, budget management, and access to community resources; travel within the District of Columbia may be required.

Qualifications

• Meet DHCF waiver case manager requirements.
• Criminal background checks completed.
• Experience with the population as specified.
• Flexible work hours and travel.

Responsibilities

• Coordinate daily operation of Medicaid Waiver services.
• Promote growth and independence of participants.
• Develop relationships with Agency staff and families.
• Ensure documentation quality and compliance with state rules.
• Abide by agency policies and procedures.
• Participate in performance improvement and safety of participants.
• Assist in forming community connections for participant inclusion.
• Support participants in developing social networks and accessing non-Medicaid resources.
• Guide participants in problem solving and decision making.
• Advocate and collaborate with other organizations on behalf of participants.
• Assist with plans (IEP, ISP, or service plan).
• Provide training on Employer of Record Model where applicable.
• Explain budget management under Employer of Record Model.
• Coordinate with Support Services provider for self-directed services.
• Provide information on recruiting, hiring, and training staff for self-directed services.
• Assist with staff scheduling and duties for self-directed services.
• Explain staff qualifications and record-keeping requirements.
