Job summary

GT Independence is seeking a Support Broker in a remote-capable role with frequent travel. You will coordinate Medicaid Waiver services, promote participant independence, and ensure proper documentation complies with state requirements.

The role emphasizes collaboration with staff, providers, and families to support client goals across community settings. Ideal candidates have a relevant degree and experience, strong interpersonal skills, and ability to manage schedules and travel.

Qualifications

• Must be 18+ years old and legally able to work in the U.S.
• Must have current licensure or the degrees listed and meet state requirements
• Background checks required per CA and federal rules

Responsibilities

• Coordinate daily operation of Medicaid Waiver services and promote participant independence
• Build relationships with staff, providers, and families
• Ensure documentation meets HCBS waiver and state requirements
• Provide training, monitor performance, and participate in quality improvement
• Enroll clients and guide ISP-based spending plans
