Job summary

GT Independence, a national leader in personal and financial services for individuals who rely on home- and community-based care, is seeking a Support Broker to coordinate the daily operation of Medicaid Waiver services and promote participant independence.

In this role you will build relationships with staff, families, and community partners; ensure compliance with state regulations; and support clients in accessing non-Medicaid resources while traveling as needed.

Qualifications

• Master’s degree in social work, psychology, counseling, rehabilitation, nursing gerontology or sociology.
• One year of experience with the population; OR bachelor’s with two years’ experience.
• RN or associate degree with three years’ experience allowed.
• Criminal background checks required by state and federal rules.
• Must be 18 years or older and eligible to work in CA.

Responsibilities

• Promote personal growth, independence, and quality of life for service participants.
• Build and maintain positive relationships with staff, providers, and families.
• Adhere to GT Mission, Values, and quality service standards.
• Ensure accuracy and timeliness of documentation per HCBS and regulations.
• Enroll clients promptly and coordinate with Independent Facilitators and Regional Center staff.
• Support participants in forming community connections and accessing non-Medicaid resources.
