ArchWell Health is seeking a Care Navigator (Bilingual) to greet and guide our members and families from arrival through departure, ensuring a positive experience and that care needs are met. The role emphasizes exceptional customer service and a passion for senior care.

The ideal candidate will welcome members, assist with check-in, manage records, coordinate referrals, and support center events, while maintaining professionalism in a dynamic health-care environment.

Qualifications

• Associates degree preferred or equivalent experience.
• Bilingual proficiency and ability to pass formal proficiency exam as part of selection.
• Minimum of one year of work experience in a clinical setting; EMR experience is a plus.

Responsibilities

• Welcome members and help them feel safe and at home, guiding them through a seamless check-in process.
• Update member information in the electronic medical records system
• Schedule appointments and coordinate referrals for a multi-disciplinary care team
• Assist members with filling out paperwork through electronic kiosks
• Request medical records and upload documentation to the EMR system
• Field questions from prospective and established members and their adult children
• Manage the center phone line, take messages, and relay details to the right person
• Assist with center events as needed
