Qualifications

• Requires deep understanding of the healthcare ecosystem, including benefits and insurance authorization.
• Must be bilingual (Spanish) and able to pass a proficiency exam.
• Minimum 2 years of referral management or care coordination experience.
• Medical assistant or medical administrative experience is helpful.
• Experience with EMR systems such as eClinicalWorks.
• Be a role model of ArchWell Health values.

Responsibilities

• Coordinate referrals to external providers, vendors, and services.
• Support members navigating care, ensuring timely appointments.
• Maintain accurate and current referral documentation.
• Build relationships with members and families to facilitate care.
• Monitor progress on care plans and follow up.
• Promote HIPAA confidentiality and clear communication.
• Be the system navigator and main contact for members and families.
• Other duties as assigned.
