Job summary

Valeris in Hartford, CT seeks a patient care coordinator to build trusted relationships with patients, explain benefits, and coordinate program services. You will handle inbound inquiries, document enrollment, and guide patients through coverage options and resources with clear, compassionate communication.

Responsibilities emphasize compliant practices, use of payer resources, and collaboration with healthcare providers to improve access and adherence to medications.

Qualifications

• Bachelor's degree strongly preferred or equivalent experience.
• Experience in customer service and healthcare industry preferred.
• Ability to work an 8.5-hour shift between 8 am to 9 pm ET, Monday to Friday.
• Bi-lingual in English and Spanish or Chinese/Mandarin is a plus.

Responsibilities

• Build and maintain trusted relationships with patients through proactive communication.
• Ensure interactions comply with healthcare laws and regulations.
• Explain benefits and coverage options to patients clearly and empathetically.
• Document patient enrollment and follow program SOPs.
• Triage patient inquiries and escalate when needed.
• Support adherence services through nurse follow-up as applicable.
