Job summary

Valeris seeks a patient-focused Care Coordinator to build relationships with patients, explain benefits, and coordinate assistance programs. The role requires navigating benefits and coverage, documenting activities, and maintaining regulatory compliance in a fast-paced setting.

The ideal candidate has strong communication, bilingual abilities, and a compassionate approach to patient care, with prior healthcare or customer service experience. Requires flexibility to work weekday hours.

Qualifications

• Bachelor's degree strongly preferred or equivalent experience.
• Customer service and healthcare industry experience strongly preferred.
• Ability to work an 8.5-hour shift between 8 am to 9 pm, Monday to Friday.
• Bi-lingual in English and Spanish or Chinese/Mandarin is a plus.
• Commitment to the mission of care coordination.

Responsibilities

• Build and maintain trusted relationships with patients by proactive communication and timely, accurate services.
• Ensure all interactions comply with healthcare laws and regulations.
• Explain benefits, coverage options, and patient responsibilities clearly.
• Document patient enrollment and maintain records in the Lynk system.
• Provide information about products, insurance coverage, costs, and financial assistance resources.
• Handle inbound inquiries per program FAQs and de-escalate when needed.
• Coordinate shipments and patient support services through program resources.
• Identify and communicate plan benefit coverage and approvals to patients and payers.
