Job summary

Elevance Health is seeking a field-based LTSS Service Coordinator fluent in Spanish and English. The role travels to client locations across numerous Florida counties to assess needs, coordinate long-term supports, and develop person-centered plans.

A BA/BS and at least 2 years in social work are required, with state-specific requirements applying. Hybrid/field work flexibility is offered in line with policy.

Qualifications

• BA/BS degree required with a minimum of 2 years of experience at a social work agency or equivalent background.
• Education and experience requirements may vary by state law and contract requirements.

Responsibilities

• Conduct face-to-face program assessments for identification and coordination.
• Coordinate members to provide comprehensive LTSS and BH/PH support in collaboration with the care team.
• Document member goals and care plans, including backup and transition plans.
• Identify members who may benefit from alternative service levels or other waivers.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to appropriate state and county agencies and participate in audits.
