Job summary

Elevance Health is seeking an LTSS Service Coordinator (Bilingual in Spanish/English) to manage a defined caseload and drive Person Centered Planning for members with LTSS/IDD, PH/BH needs. This field-based role requires travel across multiple Florida counties and residence within commuting distance of posting locations.

In this role you will conduct assessments, document goals in PCSPs, coordinate with medical directors and the interdisciplinary team, and support utilization management,

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.

Responsibilities

• Conduct face-to-face program assessments and coordinate LTSS/IDD and behavioral health needs.
• Develop Person Centered Plans and backup plans in collaboration with the member and care team.
• Interface with Medical Directors and participate in interdisciplinary care rounds.
• Submit utilization/authorization requests and report critical incidents as required.
