Elevance Health is seeking a LTSS Service Coordinator (Bilingual in Spanish/English) to support a defined caseload in the field across Florida counties. This role involves conducting assessments, developing Person Centered Plans, and coordinating with the care team to manage PH/BH/LTSS needs.

You will travel to client sites, document goals with the care team, and work with providers, caregivers, and physicians.

Formación

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.

Responsabilidades

• Perform face-to-face program assessments using predefined questions and motivational interviewing techniques for evaluations and waiver management.
• Identify members with potential clinical health care needs and coordinate cases with the clinical healthcare management and interdisciplinary team to provide care coordination support.
• Manage non-clinical needs of members with chronic illnesses to ensure cost-effective and efficient utilization of LTSS.
• Document short and long-term service goals with the member and care team, including caregivers and physicians.
• Identify members who would benefit from alternative levels of service or other waiver programs.
• Support training of new staff and participate in process improvement initiatives.
• Submit utilization/authorization requests with supporting documentation aligned to the member's care plan.
• Report critical incidents to state and county agencies as required.
