Qualifications

• BA/BS degree and minimum 2 years in social work or equivalent.
• Experience with LTSS, case management, or discharge planning preferred.
• Bilingual in English/Spanish strongly preferred.

Responsibilities

• Perform face-to-face program assessments and evaluate needs.
• Coordinate cases with clinical team to ensure integrated care.
• Manage non-clinical needs for individuals with chronic illnesses.
• Document goals and care plans with member and care team.
• Submit utilization requests and report critical incidents.
