Elevance Health is seeking a LTSS Service Coordinator (Bilingual in Spanish/English) to manage a defined caseload and perform person-centered planning. The role involves field-based assessments, coordination with medical directors, and collaboration with the member, family, and care team across multiple counties in Florida and surrounding areas.

Responsibilities include identifying needs, documenting goals, and coordinating with providers to ensure integrated PH/BH/LTSS plans.

Competenze

• BA/BS degree and at least 2 years of social work experience.
• Education/experience may vary to meet state law and contract requirements.
• Bilingual English/Spanish strongly preferred.

Mansioni

• Conduct face-to-face program assessments and coordinate LTSS/IDD, BH, PH needs.
• Serve as single point of contact to coordinate care with clinical teams.
• Document member goals and support plans with care team and caregivers.
