Job summary

Elevance Health in Florida is seeking an LTSS Service Coordinator (Bilingual in Spanish/English). This field-based role requires travel to multiple counties and occasional office attendance to support client needs and team collaboration.

You will conduct assessments, develop a Person Centered Support Plan, coordinate with medical directors and the care team, and manage non-clinical needs for members with chronic conditions.

Qualifications

• Requires BA/BS degree or equivalent experience.
• Minimum 2 years with a social work agency.
• Education/experience may be substituted per state law and contract.

Responsibilities

• Conduct face-to-face program assessments and coordinate LTSS needs.
• Develop Person Centered Plans (PCSP) and backup plans with the member.
• Serve as liaison with care team, clinicians and support circle.
• Identify members who may benefit from alternative waivers or services.
• Submit utilization and authorization requests with supporting documentation.
• Report critical incidents to state and county agencies and assist with appeals.
