Job summary

Elevance Health is seeking a Field-based LTSS Service Coordinator in Florida to manage a defined caseload with emphasis on Person Centered Planning, collaboration with care teams, and coordination of LTSS/IDD, BH, and PH needs.

The role emphasizes face-to-face assessments, flexible travel within designated counties, and partnerships with Medical Directors to develop integrated care plans while maintaining compliance with state and contract requirements.

Qualifications

• BA/BS degree + minimum 2 years of experience in a social work setting
• Education and experience requirements may vary by state law and contract requirements
• Bilingual English/Spanish strongly preferred

Responsibilities

• Conduct face-to-face program assessments using predefined questions and motivational interviewing techniques
• Coordinate and manage an individual’s waiver and BH/PH needs with the interdisciplinary team
• Document member goals in a Person Centered Planning process and maintain PCSP/back-up plans
• Identify members who may benefit from alternative levels of service or other waiver programs
• Serve as mentor or trainer for new staff and participate in process improvement efforts
• Submit utilization/authorization requests with supporting care plan documentation
• Report critical incidents to appropriate state and agency partners and participate in audits
