Elevance Health is seeking a field-based LTSS Service Coordinator who is bilingual in Spanish and English. The role focuses on conducting assessments, coordinating LTSS/IDD and BH/PH needs, and developing Person Centered Plans with the member and care team in specified Florida counties.

The position requires a BA/BS and at least 2 years of social work experience, with preference for bilingual candidates. Travel within designated counties is expected, with hybrid/onsite office discussions as

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.
• Bilingual in English/Spanish strongly preferred.

Responsibilities

• Conduct face-to-face program assessments using predefined questions and motivational interviewing techniques.
• Coordinate members' LTSS/IDD and BH/PH needs with the interdisciplinary team to develop integrated care plans.
• Document member goals and care plans with their care team and support network.
• Identify members who may benefit from alternative services or waiver programs.
• Assist with training and process improvements as needed.
• Submit utilization/authorization requests with documentation aligned to the care plan.
• Report critical incidents to state and county agencies as required.
