Qualifications

• Minimum of 2 years’ experience in senior services or related social service field.
• Excellent oral and written communication skills.
• Strong interpersonal skills.
• Ability to prioritize and handle high call volumes.
• Proficient in Microsoft Word, Excel, and Outlook.
• Must have reliable transportation, valid driver’s license and auto insurance for travel within the branch service area.

Responsibilities

• Schedule and supervise in-home care workers and clients in the geographic area.
• Conduct in-home visits to monitor and report on quality of services provided by Home Care Aides.
• Determine whether existing care plans meet client needs and adjust as necessary.
• Ensure client care plans are followed and services are not interrupted.
• Maintain schedules and attend branch meetings.
• Adhere to Medicare, Medicaid, and HIPAA regulations and company policies.
