Job summary

Addus HomeCare is seeking a driven Service Coordinator (Scheduler) to lead in-home scheduling and client coordination across a geographic area. The role focuses on ensuring quality service, timely updates, and adherence to care plans for older adults and individuals with disabilities.

You will supervise caregivers, manage schedules, perform wellness calls, and handle escalations in a fast-paced environment while maintaining confidentiality and regulatory compliance.

Qualifications

• Strong interpersonal, organizational and communication skills.
• Proficient with Microsoft Word, Excel and scheduling software.
• High school diploma or equivalent required.
• 1 year of industry experience required.
• Must have reliable transportation.

Responsibilities

• Create and manage schedules for in-home care workers and clients within a geographic area.
• Enter and adjust schedules in the system in response to client requests, illness, vacations or leaves.
• Provide alternate coverage to ensure the care plan is followed and services are continuous.
• Contact care providers and clients to give service updates.
• Conduct monthly client wellness calls and perform home visits as needed.
• Handle escalated client complaints and theft claims with thorough follow-through.
• Supervise direct service employees by setting expectations for attendance, performance and conduct.
• Maintain a professional and organized branch environment.
• Maintain confidentiality and comply with all regulations and policies.
• Be available for regular, predictable attendance and flexible to meet agency needs.
