Addus HomeCare in Albuquerque, NM is seeking a Full Time Service Coordinator to manage in-home care scheduling and oversee client services. The role involves visiting new clients, ensuring care plans are followed, and coordinating with care providers to maintain high-quality home-based care.

The ideal candidate has at least 2 years of experience in senior services, excellent communication, and strong organizational skills.

Qualifications

• Minimum of 2 years’ experience in senior services or related social service field.
• Excellent oral and written communication skills.
• Strong interpersonal skills and customer service orientation.
• Ability to prioritize and handle high call volumes.
• Proficient in Microsoft Word, Excel, and Outlook.
• Must have reliable transportation and valid driver's license for travel within service area.

Responsibilities

• Schedule and supervise in-home care workers and clients in the service area.
• Conduct in-home visits to new clients and review Welcome Packet.
• Monitor care plans and ensure they are followed; update as needed.
• Provide service updates to care providers and clients.
• Maintain compliance with Medicare, Medicaid, and HIPAA regulations and standards.
• Participate in weekly branch meetings and ensure confidentiality.
