Addus HomeCare, Inc. in Albuquerque, NM is seeking a Full Time Service Coordinator to manage schedules and supervise in‑home care workers and clients in the local area. You will conduct home visits, monitor care plans, and report on service quality to ensure client safety and independence.

Ideal candidates have a minimum of 2 years’ experience in senior services, strong communication, and proficiency with MS Office; travel within the service area is expected.

Qualifications

• Minimum 2 years’ experience in senior services or related social service field.
• Excellent oral and written communication skills.
• Reliable transportation with valid driver’s license and insurance for travel within the branch service area.

Responsibilities

• Create and manage work schedules; adjust for client needs and absences.
• Provide alternate coverage to follow care plans without interruption.
• Contact care providers and clients for service updates.
• Monitor client condition and aides’ performance.
• Conduct home visits to welcome new clients and review Welcome Packet.
• Ensure compliance with Medicare, Medicaid, and HIPAA regulations.
• Prepare evaluation reports and maintain confidentiality.
