Job summary

Wider Circle is seeking a Bilingual Community Engagement Specialist to lead Connect For Life in Lancaster, PA—with Reading and York considered. You will host virtual and in-person events, engage members one-on-one and in groups, and build trusted relationships to improve health outcomes.

Candidates should have 3+ years in outreach, knowledge of health systems, and a preference for a PA CHW certificate. Role includes home visits, event planning, and collaboration with diverse community members to

Qualifications

• 3+ years in community outreach, teaching, or facilitation. Healthcare experience is a plus.
• Pennsylvania Community Health Worker Certificate strongly preferred.
• Associate's degree preferred; 5+ years of relevant experience may be considered instead of a degree.

Responsibilities

• Lead the Connect For Life program, building trust and promoting health.
• Plan and host virtual and in-person events to foster relationships and wellness outcomes.
• Proactively contact members, conduct home visits, and facilitate monthly engagement.
• Record member interactions and attendance in Wider Circle's digital systems.
• Identify natural leaders to serve as Wider Circle Ambassadors and grow communities.
• Outreach to prospective members and develop enrollment plans.
• Serve as a liaison, connecting members to health plans and care resources.
• Monitor engagement metrics and report on hours and outcomes.
• Operate effectively in a remote environment with minimal supervision.
