Wider Circle is seeking a Bilingual Community Engagement Specialist to lead Connect For Life in the Lancaster area, serving Reading and York borders. You will run events, conduct in-home visits, and engage neighbors to improve health outcomes.

You will build trust, coordinate virtual and in-person activities, and proactively reach out to members, while maintaining thorough documentation and reporting. This role requires flexibility, travel, and strong communication.

Qualifications

• Bilingual English/Spanish required.
• 3+ years in community outreach, teaching, or facilitation.
• Pennsylvania Community Health Worker Certificate strongly preferred.
• Education: Associate's degree preferred; substantial experience may substitute.
• Reliable transportation for home visits; travel 20–50 miles.

Responsabilités

• Lead the Connect For Life program, building trust and promoting health.
• Plan and host virtual and in-person events to foster member relationships.
• Proactively contact members and conduct home visits to boost engagement.
• Document interactions in Wider Circle's digital systems in a timely manner.
• Identify natural leaders and invite them to serve as Wider Circle Ambassadors.
• Outreach to enroll new members and connect them to health plans and resources.
• Monitor engagement metrics and report on attendance and outcomes.
• Volunteer as liaison with health plans to close care gaps.
• Maintain accountability for membership growth and engagement goals.
• Conduct home canvassing and flexible scheduling as needed.
