Wider Circle in Charlotte, NC is seeking a Bilingual Community Engagement Specialist to lead Connect for Life, a program that connects neighbors for better health through events, home visits, and outreach. You will recruit and engage members, plan virtual and in-person activities, and build trusted relationships within the community.

You will track participation, report outcomes, and collaborate with health plans and providers to close care gaps, while maintaining a proactive and adaptable

Qualifikationen

• 3+ years in community outreach, teaching, or facilitation.
• Associate degree preferred; 5+ years may be considered in lieu of a degree.
• Strong communication and networking skills.

Aufgaben

• Lead Connect For Life program with events, virtual and in-person interactions.
• Plan, host and outreach to members for engagement and attendance.
• Proactively contact members and conduct home visits to build relationships.
• Record interactions and track engagement in online systems.
• Identify community leaders and invite ambassadors to support events.
• Coordinate with health plans and providers to close care gaps.
