Wider Circle is seeking a Bilingual Community Engagement Specialist in Lancaster, PA to lead the Connect For Life program and build a thriving neighbor network focused on health and wellbeing. You will plan and host virtual and in-person events, conduct home visits when needed, and actively engage members to meet care needs while expanding the community.

Qualifikationen

• Pennsylvania Community Health Worker Certificate strongly preferred.
• 3+ years in community outreach, teaching, or facilitation.
• Healthcare experience is a plus.
• Associate’s degree preferred; 5+ years of relevant experience may be considered instead of a degree.

Aufgaben

• Lead the Connect For Life program, building trust and promoting health.
• Plan and host virtual and in-person events to foster relationships and wellness outcomes.
• Proactively contact members, conduct home visits, and facilitate monthly engagement.
• Record member interactions for tracking and reporting in Wider Circle systems.
• Identify natural leaders to serve as Wider Circle Ambassadors and grow communities.
• Develop personalized plans to enroll potential members in Connect For Life.
• Serve as liaison connecting members to health plans and care services.
• Achieve membership engagement and growth goals with time-tracking and reporting.
