Job summary

21st Century Home Health Services in California seeks an Occupational Therapist to evaluate and address patients in their homes, enabling safe, productive living. You will partner with the Case Manager to address home safety, medication management, wheelchair seating and positioning, pain management, falls prevention and dementia care.

Qualifications

• Current unrestricted CA Occupational Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources is required.
• Knowledge of state and federal homecare regulatory guidelines preferred.
• Experience home health care is highly preferred
• Experience with high acuity patients is a plus
• Experience working with an interdisciplinary team is highly desired
• Case Management fundamentals is highly desired

Responsibilities

• Evaluate and address needs of individuals in their homes so they can live safely and productively at home
• Address home safety, medication management, wheelchair seating and positioning, pain management, falls prevention
• Manage chronic diseases and dementia family caregiver training
• Educator, patient advocate and member of the interdisciplinary care team
