Job summary

21st Century Home Health Services seeks an Occupational Therapist to join our Sacramento County/Roseville territory. You will provide skilled in-home evaluations and treatments to help patients live safely at home, working with the interdisciplinary care team.

In partnership with the Case Manager you will address home safety, seating and positioning, pain management, falls prevention, and chronic disease care.

Qualifications

• CA OT license required in good standing.
• Current BLS/CPR certification.
• Valid CA Driver's License.
• Home health care experience preferred.

Responsibilities

• Provide skilled in-home evaluations and treatments to help patients live safely at home.
• Collaborate with Case Manager and interdisciplinary care team to address home safety, seating/positioning, pain management, falls prevention, and chronic disease care.
• Educate patients and caregivers; advocate for high-quality, compassionate home health care.
