Job summary

21st Century Home Health Services Inc. is seeking a Full Time Occupational Therapist to serve in Hayward, CA. You will work with the Case Manager to deliver skilled OT services in patients' homes, evaluating needs and implementing plans to improve safety and independence.

Requirements include a CA OT license, BLS/CPR, and a valid CA driver’s license. We offer a flexible schedule, generous benefits, and a performance-based bonus potential.

Qualifications

• Current unrestricted CA Occupational Therapist License.
• BLS/CPR certification required.
• Valid CA Driver's License.
• Experience in home health care preferred.
• Ability to work in a field-based home health setting.
• Knowledge of community resources and care planning.

Responsibilities

• Provide skilled OT services in patients' homes in partnership with the case manager and care team.
• Evaluate home safety, mobility, and caregiver training.
• Develop and implement patient-centered treatment plans.
• Educate patients and families on safety and independence.
• Coordinate with the interdisciplinary team to optimize outcomes.
