Job summary

21st Century Home Health Services (21HHS) seeks an Occupational Therapist to provide in-home OT care in California. You will be assigned a primary location with occasional coverage in surrounding areas, visiting patients in their homes as part of a growing organization.

The role requires a CA OT license, BLS/CPR, and a valid CA driver’s license. Competitive compensation, a 401(k) with up to 4% company match, and comprehensive benefits are offered.

Qualifications

• Current unrestricted CA Occupational Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources is required.
• Experience home health care is highly preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals is highly desired.
