21st Century Home Health Services in the Bay Area seeks an Occupational Therapist to provide in-home skilled OT in Alameda County (Berkeley). You will evaluate needs, address home safety, seating, pain management, and dementia training as part of the interdisciplinary care team.

You will educate patients and families, drive safe outcomes, and enjoy flexible schedules, autonomy, and strong benefits including 401k match, generous PTO, and health coverage.

Qualifications

• Current unrestricted CA Occupational Therapist License.
• BLS/CPR certification required.
• Valid CA Driver's License.

Responsibilities

• Provide skilled OT services in patients' homes, enabling safe, productive living.
• Collaborate with Case Manager and interdisciplinary care team.
• Educate patients and families, advocate for care and safety.
