Job summary

21st Century Home Health Services is seeking an Occupational Therapist to join its field-based home health team in California. You will provide OT assessments, plan of care, and in-home therapy visits to patients.

The role requires CA OT license, BLS/CPR, CA driver’s license, and the ability to work with an interdisciplinary team, with comprehensive benefits and a competitive salary. You may be assigned a primary location with occasional coverage in surrounding areas as patient needs arise;

Qualifications

• Current unrestricted CA Occupational Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
• Experience in home health care and with high-acuity patients is a plus.
• Experience with interdisciplinary teams is highly desired.

Responsibilities

• Provide OT home health services to patients in assigned territory.
• Collaborate with an interdisciplinary care team.
• Manage patient cases and coordinate care.
• Adhere to regulatory guidelines and quality standards.
• Travel to patient homes as required.
