Job summary

21st Century Home Health Services is seeking an Occupational Therapist to deliver skilled home health services in the Salinas area. You will evaluate needs, address home safety, and collaborate with a Case Manager as part of an interdisciplinary team to support safe, independent living for patients at home.

Requirements include a CA OT license, BLS/CPR, and a CA driver’s license. Experience with high-acuity cases and home health is preferred; strong interpersonal skills and organization are

Qualifications

• Current unrestricted CA Occupational Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources is required.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.

Responsibilities

• Provide skilled OT services in patients' homes, evaluating needs and promoting safe, productive living.
• Educate patients and caregivers; act as advocate and member of the interdisciplinary care team.
• Collaborate with the Case Manager to coordinate care and services.
• Address home safety, medication management, wheelchair seating/positioning, and falls prevention.
