Job summary
21st Century Home Health Services is seeking an Occupational Therapist (OT) to join our Alameda County home health team in Emeryville, CA. You will collaborate with the Case Manager to deliver skilled OT services in patients' homes, promoting safety and independence.
The ideal candidate holds a CA OT license, BLS/CPR, and a CA driver’s license, with experience in home health and interdisciplinary teams. We offer flexible schedules, autonomy, and competitive benefits.
Qualifications
- CA OT license (unrestricted) required.
- Current BLS/CPR certification.
- Valid CA driver's license.
- Knowledge of local community resources.
- Experience in home health and high-acuity patients preferred.
- Experience with an interdisciplinary team.
Responsibilities
- Provide skilled OT services in patients' homes in partnership with Case Manager.
- Evaluate and address home safety, seating/positioning, pain management, falls prevention.
- Educate patients and families; advocate and participate in the care team.
- Support dementia care, chronic disease management, and caregiver training.
