Job summary

21st Century Home Health Services in Morgan Hill, CA, seeks an Occupational Therapist to provide in-home skilled OT services and improve safety and independence for patients on their care plan.

You will work with Case Managers to address home safety, pain management, seating/positioning, and chronic disease management, while educating families and collaborating with an interdisciplinary team.

Qualifications

• Current unrestricted CA Occupational Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources is required.
• Knowledge of state and federal homecare regulatory guidelines preferred.
• Experience home health care is highly preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals is highly desired.

Responsibilities

• Provide skilled OT services in patients' homes to evaluate and address safety and functional needs.
• Support home safety, medication management, wheelchair seating/positioning, pain management and falls prevention.
• Collaborate with the interdisciplinary care team to manage chronic diseases and dementia in the home setting, including family caregiver training.
