Job summary

21st Century Home Health Services (21HHS) is seeking a Full Time Occupational Therapist to visit patients in homes in the Galt area. You will evaluate and address needs to help individuals live safely and productively at home.

In collaboration with the Case Manager and interdisciplinary team, you will deliver skilled services, educate caregivers, and support independent living with a patient- and family-centered approach. Flexible schedules and strong benefits are offered.

Qualifications

• CA Licensed Occupational Therapist (OT) with CA license.
• Current BLS/CPR certification.
• Able to drive in CA with valid license.
• Experience in home health care is highly preferred.

Responsibilities

• Provide skilled OT services in patients' homes.
• Evaluate needs and address home safety, mobility, and daily function.
• Educate patients and caregivers; collaborate with the interdisciplinary team.
