Heartland Hospice is seeking an After Hours / On-Call Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays. You will provide direct patient care, coordinate with interdisciplinary team, and ensure seamless handoffs with daytime staff.

Ideal candidates hold an active RN license in Iowa, have 1+ year nursing experience, CPR certification, and a valid driver's license. This role involves travel to patient homes and requires compassionate, calm decision-making in

Formación

• Current RN license in IA or eligibility to obtain
• 1+ years nursing experience
• CPR certification
• Valid driver’s license and reliable transportation

Responsabilidades

• Provide direct patient care after normal business hours
• Serve as primary liaison between patients, families, physicians and hospice team during shift
• Respond promptly to urgent calls, patient visits, and symptom management needs
• Assist in developing and implementing the Plan of Care with the Interdisciplinary Group
• Document visits and communications accurately and maintain chart readiness
• Participate in quality assessment and performance improvement programs
• Coordinate with daytime care team to ensure continuity of care
