Heartland Hospice is seeking an After Hours / On-Call Hospice RN in Iowa to respond to patient needs during evenings, nights, weekends, and holidays. You will provide direct patient care, coordinate with an interdisciplinary team, and ensure continuity of care in patients’ homes or facilities.

The role requires an active RN license, 1+ year nursing experience, CPR certification, and a compassionate, calm demeanor for emotionally intense settings.

Qualifications

• Current RN license in the state of employment
• 1+ years of nursing experience
• Hospice mindset: compassionate, calm, dependable in urgent care
• Excellent clinical judgment and emotional support in intense settings
• Comfort delivering care in patients' homes or facilities
• Valid driver’s license, auto insurance, reliable transportation
• Current CPR certification

Responsabilités

• Respond to patient needs during evenings, nights, weekends, and holidays
• Provide direct patient care and support after normal business hours
• Serve as the primary liaison between patients, families, physicians, and hospice team
• Assess patient needs and guide clinical decisions per the plan of care
• Document visits and communications accurately
• Collaborate with interdisciplinary team to provide holistic care
• Participate in quality improvement programs and maintain records
• Ensure continuity of care with daytime team and safety adherence
