Job summary

Heartland Hospice is seeking an After Hours RN to provide compassionate, in-home hospice care during evenings, nights, weekends, and holidays in Hubbard, IA. You will respond to urgent needs, document visits, and coordinate with the care team to ensure seamless continuity of care.

The role requires an active RN license in the state, 1+ year nursing experience, valid CPR, and reliable transportation. You will work with an interdisciplinary team to support patients and families during challenging

Qualifications

• Current RN license in the state of employment (or eligibility to obtain)
• 1+ years of nursing experience
• A hospice heart: compassionate, calm, and dependable in urgent care situations
• Excellent clinical judgment and the ability to provide support in emotionally intense settings
• Comfortable providing care in patients’ homes, facilities, or wherever they reside
• Valid driver’s license, automobile insurance, and reliable transportation
• Current CPR certification

Responsibilities

• Respond to patient needs after hours, evenings, weekends, and holidays
• Serve as liaison between patients, families, physicians, and the hospice team
• Document clinical visits and communications in the EMR
• Evaluate patient and family needs to guide care decisions and plan of care
• Collaborate with the Interdisciplinary Group to provide comprehensive care
