Job summary

Heartland Hospice is seeking an After Hours RN to deliver compassionate, patient-centered care in patients' homes during evenings, nights, weekends, and holidays. You’ll collaborate with the hospice team, document visits, and adjust plans of care as needs evolve.

A current Iowa RN license, 1+ year nursing experience, CPR certification, and reliable transportation are required. You will report to the Patient Care Manager and work with LPNs/LVNs, CNAs, and other staff to ensure high-quality

Qualifications

• Current RN license in the state of employment.
• 1+ years of nursing experience.
• Current CPR certification.
• Strong EMR/documentation skills.
• Preferred hospice/home health experience.

Responsibilities

• Provide direct patient care and support after-hours.
• Serve as liaison between patients, families, physicians, and hospice team.
• Document clinical visits and care plans in EMR.
• Respond to urgent calls and symptom management needs.
• Collaborate with interdisciplinary team and daytime care staff.
• Participate in quality assessment and performance improvement programs.
