Job summary

Heartland Hospice is seeking an After Hours/HOME-BASED RN to respond to patient needs during evenings, nights, weekends, and holidays. You will be the primary liaison between patients, families, and the hospice team, providing direct care and symptom management after hours.

You will document activities in EMR, collaborate with LPNs, CNAs and other staff, and participate in quality improvement initiatives to ensure continuity of care with the daytime team.

Qualifications

• Current RN license in the state of employment.
• 1+ years of nursing experience.
• CPR certification.
• Familiarity with EMR systems and documentation practices.

Responsibilities

• Respond to patient needs after normal business hours.
• Serve as the primary liaison between patients, families, physicians and hospice team during shift.
• Provide direct patient care and symptom management as needed.
• Document visits and communications accurately in medical records.
• Participate in quality improvement programs and maintain continuity of care with daytime team.
