Job summary

Heartland Hospice is seeking an After Hours / On-Call Registered Nurse to respond to patient needs during evenings, nights, weekends, and holidays. This role provides direct patient care, serves as a liaison among patients, families, physicians and the hospice team, and ensures timely documentation and continuum of care.

Candidates should have an active RN license, CPR certification, and at least 1 year of nursing experience.

Qualifications

• Current RN license in the state of employment.
• 1+ years of nursing experience.
• Current CPR certification.

Responsibilities

• Respond to patient needs during after-hours.
• Serve as liaison between patients, families, physicians and hospice team.
• Document visits and communications.
• Participate in quality assessment and improvement programs.
• Coordinate with daytime care team.
