Job summary

Heartland Hospice is seeking an After Hours / On-Call Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays. You will act as the primary liaison between patients, families, physicians, and the hospice team, delivering direct patient care and documenting visits in the EMR system.

You will assess patient and family needs, guide clinical decisions, and participate in quality improvement programs, all while coordinating with the daytime care team to ensure continuity

Qualifications

• Current RN license in the state of employment or eligibility to obtain
• 1+ years of nursing experience
• Compassionate, calm and dependable in urgent care situations
• Excellent clinical judgment and ability to provide support in emotionally intense settings
• Comfortable providing care in patients' homes or wherever they reside
• Valid driver’s license, automobile insurance, and reliable transportation
• Current CPR certification

Responsibilities

• Respond to patient needs after normal business hours (evenings, nights, weekends, holidays)
• Serve as primary liaison between patients, families, physicians, and the hospice team
• Document clinical visits and communications accurately
• Evaluate patient and family needs to guide clinical decisions
• Participate in quality assessment and performance improvement programs
• Collaborate with interdisciplinary team to ensure continuity of care
• Maintain up-to-date medical records and adherence to plans of care
• May perform routine visits during normal business hours if needed
