Heartland Hospice is seeking an After Hours RN to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care where it is needed most. You will work independently, document visits, and coordinate with the daytime team to ensure continuity of care.

You will collaborate with LPNs, CNAs, and other staff, assess patient needs, and implement the plan of care while maintaining high professional standards and comprehensive documentation.

Qualifications

• Current RN license in the state of requested employment (or eligibility to obtain).
• 1+ years of nursing experience.
• Hospice heart: compassionate, calm, and dependable in urgent care situations.
• Excellent clinical judgment and the ability to provide support in emotionally intense settings.
• Comfortable providing care in patients’ homes, facilities, or wherever they reside.
• Valid driver’s license, automobile insurance, and reliable transportation.
• Current CPR certification.

Responsabilités

• Respond to patient needs after hours and on-call.
• Serve as liaison between patients, families, physicians, and hospice team.
• Document visits and communications accurately.
• Evaluate needs and implement the plan of care.
• Collaborate with interdisciplinary team including CNAs and LPNs.
