Job summary

Heartland Hospice is seeking an After Hours / On-Call Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays. You will serve as the primary liaison between patients, families, physicians, and the hospice team, and you will document visits and coordinate with the daytime care team.

This role requires an active RN license in the state of employment and at least 1 year of nursing experience, with CPR certification.

Qualifications

• Current RN license in the state of requested employment (or eligibility)
• 1+ years of nursing experience
• Current CPR certification

Responsibilities

• Respond to patient needs during evenings, nights, weekends, and holidays
• Serve as the primary liaison between patients, families, physicians, and the hospice team
• Document clinical visits and communications
• Participate in quality assessment and performance improvement programs
• Work independently while coordinating with daytime care team
